[Green] sustainability audit

Not sure why it was a problem but this version should be fine.  I did 
not want to add more questions so I incorporated most of the suggestions 
into the other questions and added one on food consumption.  Rebecca, if 
you can get this into the Google form then I can include the link for 
the survey to Adeline so it can be in the next newsletter.  I am hoping 
it's possible to have an addition to the newsletter (at the end perhaps) 
for people who might need to find it that way and not search the 
Internet.  Annie
