[Green] sustainability audit

Here is the final survey version.  I am hoping to hear from Dawn so I 
can get it off to Adeline for the newsletter (I hope she can attach it 
at the end of the newsletter).  The plan is for it to appear twice in 
the newsletter, several times on FUUSM's FB page and a couple of 
broadcast emails (sent by Chris).  Other marketing ideas to get people 
to participate are welcome?  We are offering a plant (value up to $25) 
from the annual plant sale in a drawing for all surveys returned.  Annie
