[Green] My input to survey

[Annie Warmke]

The goal of the survey is to distribute widely and then to gather the 
results so that they can be used to influence public policy and 
community direction.  Any edits are greatly appreciate, and if you can 
guide us in the gathering of the data that would be helpful too.
