[Fuusm-l] Survey on the UUA Amendments - fill out in google forms

I just sent you a short survey. Unfortunately, I opted to include the text
of the survey in the email which might confuse you.  To answer the survey
you click on the box that says Fill Out in Google Forms.  It only takes a
few seconds.

Look for this box:
